Mandetory Birth Certificate Requirement

If you are new to the blaze, you MUST furnish a birth certifcate prior to any equipment being provided. You will not be alllowed to play without providing a birth certificate.

Here are the two options for submitting birth certificates:

1) Email a copy of your birth certificate to administrator@brentwoodblaze.org.

Or

2) Bring a copy of the birth certificate to equipment pick up.
